attempt to reflect the fact that almost two-thirds of
women with post caesarean pregnancy can deliver by
vaginal route given an adequate trial of labour with good

Kaposi's sarcoma is an unusual neoplasm, primarily
involving the skin and especially over the lower
extremeties. It is prevalent in Africa south of the
Sahara, and is a common occurrence in young homosexual
males. In this article, a case of Kaposi's sarcoma
occurring as a secondary malignancy in a patient
receiving treatment for lymphoepithelioma is reported.

author first saw Oman) mothers no longer tell you that

children, are all real indicators that child survival in
Oman has changed in the last ten years.

At a time when both health care and research capable of
improving it have become increasingly expensive,
spontaneous evolution of health system for children and
their mothers becomes unlikely. In an effort to make the
delivery of health care for babies and their mothers more
efficient and effective, a follow-up clinic was
envisioned in Sumail Hospital last December, to give a
holistic approach in the delivery of the basic health
services for newborn babies and their mothers, a clinic
suitable for tackling the basic problems responsible for
the majority of all children's deaths and illnesses such
as poor nutritional health, malnutrition-related
infections, diarrhoea and the six communicable diseases.
It was formally launched in January 1, 1986, coinciding
with the nationwide programme of the Ministry of Health
for comprehensive child and mother care. A 6-month trial
study has been done and the following results, problems
encountered, and recommendations are presented.

Gallstone ileus is a rare cause of intestinal
obstruction, which occurs due to luminal impaction of
gallstone in the small bowel. It was first described by
Barthion in 1654. Courvoisier reported 131 cases of
gallstone ileus in 1890, with a mortality rate of about
44 percent. It is responsible for one to three percent of
all mechanical obstructions and occurs exclusively in
elderly persons. As the symptoms are elusive, diagnosis
is often difficult and usually delayed. A case of
gallstone ileus, diagnosed pre-operatively is reported.

When confronted by a patient with cholestasis, the over-
riding objective is to determine whether a potentially
treatable cause of cholestasis is present without
subjecting the patient to needless risk, or discomfort.
Clinical judgement plays an exceedingly important role in
the approach to the cholestasic patient. The initial task
of the clinician is to determine whether the patient is
or is not likely to have biliary obstruction. If biliary
obstruction is not likely, then an invasive work-up is
usually not indicated. In contrast, most causes of
biliary obstruction are not self-limited and definitive
or palliative treatment requires active intervention.

Two cases of chondrosarcoma in rather flat bones, along
with the clinical findings, radiological appearances and
microscopic features are presented. Chondrosarcoma can be
classified into four types; A. Primary -Low grade arising
in adult life; B. Secondary - Malignant transformation of
benign cartilagenous lesion. From Enchondroma of
Exostosis; C. Mesenchymal - Rare, seen in skull, spine,
ribs or pelvis; D. Dedifferenciated - Rare, but in long
bones.

An unusual case of cryptorchism is presented in which
both the testes were on the same (right) side. One of
them was normally placed in the scrotum while the other
was in the ipsilateral inguinal canal, alongwith an
indirect inguinal hernia. The left side of his scrotum
was empty.

Cytomegalovirus (CMV) infection in infancy is well
documented though rarely recognised. We describe an
infant presenting with cataracts and persistent asites
associated with CMV hepatitis.

Seroprevalence of rubella, cytomegalovirus, measles and
toxoplasmosis among pregnant women in Oman. February

Serum samples collected from pregnant women in different
regions of Oman were tested for IgG antiboides against
rubella and measles viruses, cytomegalovirus (CVM) and
Toxoplasma gondii by enzyme-linked immunosorbent assays.
None of these women had been vaccinated against measles
and rubella.
The results show that most of Omani women of childbearing
age have had rubella, CMV and measles infection. In
addition, the data suggest that natural immunity to
rubella in this country is commonly acquired before
puberty.
