"Fuad" and "Fuadi" are colloquial terms used to describe,

and are characterised by, symptoms like epigastric pain,

discomfort, palpitations, dyspnea of acute onset and

signs suggestive of peptic ulcer disease, hypertensive

heart disease, myocardial ischemia and many other

symptoms/signs. "Fuad" means "heart" and "Fuadi" means

"my heart". The purpose of this study was to make an

attempt to reduce the existing confusion by analysing the

patients who attended the OPD with the complaints of

"Fuad" and match them with the underlying pathology. On

an average 14,000 to 15,000 patients attended the OPD of

Sumail Hospital, each month, and of whom 30 to 40% (about

4,500) with Fuad syndrome, and consume around 75kg of

antacids within and outside the hospital.

Bleeding from oesophageal is one of the most serious and

often life threatening forms of upper gastrointestinal

haemorrhage. One third of the patients with cirrhosis do

not survive the first haemorrhage and Olsson did not find

any survivors among the patients with child C liver

disease six months after the first haemorrhage.

Management of variceal haemorrhage has been a most

frustrating and persistant challenge to physicians and

surgeons. Recently endoscopic sclerotherapy is becoming

popular for both acute bleeding episodes and achieving

long term obliteration of oesphageal varices. Most

reports of long term sclerotherapy come from institutions

where alcohol or schistosomiasis are the major causes of

cirrhosis. Experience of sclerotherapy in Oman where

hepatitis B is the major cause of cirrhosis is presented.

A sixty-year-old man was admitted to Qaboos Hospital,

Salalah, on 13th July 1985 with a three-day history of

epigastric pain and vomiting. The pain was described as a

sharp severe pain which was not radiating and which came

on after heavy work. The initial vomit was yellowish

green which later become heavily blood stained. Two years

ago he was operated upon for chronic ulcer in Cairo. No

details were available regarding the nature of his

operation. Patient had been otherwise healthy during his

life time.
