The referral form has been updated. Below are the new main features. The links to the forms appear at the end.

New Features

Resolutions sent via Email

After submitting a student referral, teachers will receive the CPS’ resolution through their SQU email.


Follow-up on Referrals

If teachers need to follow-up on a referral, a link for that is provided at the end of the CPS’ email.


How to follow-up:

1.      Click on the link at the end of the CPS’ email.

2.      The link takes you back to the referral form.

3.      Click on “Follow-up on a submitted referral”.

4.      Choose the reason for your follow-up.

5.      Provide brief details.

6.      Click “Submit”.
