Inpatient /Clinical pharmacy
Clinical Pharmacists work in teams with other health care providers to promote rational drug therapy that is safe, appropriate and cost effective. They are responsible for regular reviewing of in patient medication charts and patient files. This is to ensure the safety and effectiveness of prescribed medicines. They contact physician and provide appropriate options when they have a concern about the choice or safety of the prescribed medicines. They also answer specific requests from medical and nursing staff regarding different medication or administration methods and provide patients with the required information related to their medicines.
